Programme 1: Health Services (Disease Management of HIV/AIDS and other Chronic Diseases)
Objective 1: Through a range of targeted health and wellness support services, assist the Joe Slovo Community to better manage the impact and reduce the loss of lives associated with the high burden of disease (particularly HIV/AIDS).
Outcome 1
Home Based Care Services
Provide quality Home Based Care (HBC) services to those in need in the Joe Slovo Township
Outcome 2:
Treatment Adherence/Tracing Defaulters
Assist Clinic to achieve National objectives by ensuring Treatment Adherence/ Tracing of Defaulters
Outcome 3:
Medical Testing (HIV/TB)
provided in homes
Outcome 4:
Psycho-social Support (Individual)
Refer individuals in need (HIV and other) to Counselling Services
Outcome 5:
Psycho-social Support (Group)
access is provided to people living with HIV/Aids (PLWH) through facilitation of Support Groups
Outcome 6:
Health and Wellness Education
provided to HIV +ve clients to ensure better health outcomes.
Outcome 7:
Nutrition/Food Security
provided to clients and/or those in need of nutritional support

Programme 2: Health Communication (Prevention: HIV, STI's, TB and Wellness)
Objective 2: Through effective and well-coordinated Health & Wellness Communication, prevent the transmission of HIV&AIDS and other diseases and improve overall wellness knowledge in the community

Communications Programme
Community receives consistent & clear health messages through coordinated efforts.
Outcome 2:
Behaviour Change and Testing
Encourage health seeking behaviour/health communication brings about desired behaviour change.

Programme 3: Children's Services (OVC's)
Objective 3: Identify and assist children at Primary School level who are in need of basic care, food security, and education including literacy/numeracy and accessing social support services.

Nutritional support
Assist OVC's in need of food support
Outcome 2:
Educational Support
Assist OVC's with educational support with after school literacy/numeracy program and by providing recreational/sports activities.
Outcome 3:
Health and Social Services
Ensure OVC's access health and social services including documents/grants and counselling, clinic for health issues.

Programme 4: Advocacy - Accessing Social Services and Improving Livelihoods.
Objective 4: Increase access to essential health and social services and strive for improved livelihoods by advocating for the community on broader community issues

Assist community to access essential services including official documentation and grants.
Outcome 2:
Represent community on decision making structures
relating to health, improving livelihoods and risk management. Participate in Disaster Risk Preparedness programmes

Generate Opportunities
Assist community to gain from opportunities for income generation and skill development.
